What Is Tirzepatide?
Tirzepatide is a 39-amino-acid synthetic peptide that activates both glucagon-like peptide-1 and glucose-dependent insulinotropic polypeptide receptors. It is based on the native GIP sequence and modified with a C20 fatty diacid chain that promotes albumin binding and extends its half-life to approximately five days.
It is FDA approved as Mounjaro for type 2 diabetes and as Zepbound for chronic weight management. Zepbound is also approved for moderate-to-severe obstructive sleep apnea in adults with obesity.
The dual mechanism separates tirzepatide from single-receptor GLP-1 agonists. GLP-1 activity primarily suppresses appetite, slows gastric emptying, and improves glucose regulation, while GIP activity adds insulinotropic effects and may improve nutrient partitioning and overall tolerability.

In SURMOUNT-1, mean weight reduction reached 15.0% at 5mg, 19.5% at 10mg, and 20.9% at 15mg over 72 weeks.
Compared with Semaglutide
In SURMOUNT-5, maximum-tolerated tirzepatide produced 20.2% mean weight reduction versus 13.7% with semaglutide at 72 weeks.
Body Composition
A SURMOUNT-1 DEXA analysis found that approximately 75% of weight lost was fat mass and 25% was lean mass.
Blood Sugar Control
SURPASS trials reported HbA1c reductions of up to approximately 2.3 percentage points, with stronger results than semaglutide 1mg in SURPASS-2.
Liver Fat & MASH
In the phase 2 SYNERGY-NASH trial, MASH resolution without worsening fibrosis occurred in 44%–62% of tirzepatide-treated participants versus 10% with placebo.
Sleep Apnea
In SURMOUNT-OSA, mean apnea-hypopnea index changes were −25.3 and −29.3 events per hour with tirzepatide versus −5.3 and −5.5 with placebo.
