What is NAD+?

NAD+ (nicotinamide adenine dinucleotide) is a molecule found in every cell of your body that plays a central role in energy production, DNA repair, and cellular health. It is not a peptide. It is a coenzyme, which means it helps enzymes do their jobs.

Without adequate NAD+, your cells cannot produce energy efficiently, repair damaged DNA, or maintain the hundreds of metabolic reactions that keep you functioning.

Most people hear about NAD+ as an anti-aging supplement, but before deciding whether it is relevant, it helps to understand what it does at the cellular level, what happens when it declines, and the different ways to address that decline.

The Fuel Analogy

NAD+ is fuel. SS-31 is the mechanic. MOTS-c is the supercharger. If the mitochondrial machinery is damaged, adding more fuel may only get you so far. Repair structure first, then support fuel and optimization pathways.

Why NAD+ Matters for ATP

Every single thing your body does requires energy, and that energy comes from ATP. Your cells use ATP to contract muscles, fire neurons, repair tissue, and keep you alive.

The body maintains only a small immediately available ATP pool relative to its daily energy demand. ATP therefore has to be regenerated continuously throughout the day.

The NAD+/NADH redox pair is essential to that process, carrying electrons through metabolic pathways that feed the electron transport chain. When cellular NAD metabolism is impaired, energy production and other NAD-dependent processes can be affected.

NAD+ Decline With Age

Human studies report age-related declines in NAD+ across several tissues and blood measurements, although the size and timing of the decline vary by tissue, health status, and measurement method.

Lower NAD+ availability is associated with impaired cellular energy metabolism, reduced repair capacity, metabolic dysfunction, and several biological features of aging.

Side Effects

NR has been studied at doses up to 2000mg daily, while human NMN trials have used varied and generally lower doses. Both appear generally well tolerated in short-to-medium studies, but injectable NAD+ has far less standardized safety data and is commonly reported to cause more local discomfort.

Long-term safety data beyond 12 months remains limited, and injectable NAD+ protocols vary substantially in formulation, concentration, and supervision.

NAD+ is commonly described as more uncomfortable to inject than typical peptides. A burning or stinging sensation lasting around 20 minutes is frequently reported. Injecting slowly and using buffered NAD+ may reduce discomfort.

Contraindications and Precautions

Do Not Use If You Have

  • Active cancer, because NAD metabolism can support both normal and malignant cells and the safety of direct NAD+ supplementation in active cancer is not established.
  • Severe liver disease, because the liver is involved in NAD+ metabolism.
  • Severe kidney disease, because the kidneys help clear NAD+ metabolites.
  • Known hypersensitivity to NAD+ or any component.

Use Caution With

  • Pregnancy or breastfeeding, due to lack of safety data.
  • Bleeding disorders if using an injectable form.
  • Diabetes medications or metformin, because formal interaction data with injectable NAD+ is limited.

Drug Interactions

Niacin—not NAD+ itself—may interact with blood pressure medications, blood thinners, and diabetes medications. Formal interaction data for injectable NAD+ remains limited, and no well-established interaction pattern has been defined.

FINAL VERDICT

Peptide Protocol's Opinionated Opinion

This is not medical advice. This editorial is our own opinion piece based off the experience of our own lab rats.

We think NAD+ is probably a bit overrated. It does work for many people, but the mileage does seem to vary quite a bit per person. Some get a very noticeable boost in energy that lasts 4-6 hours, while others feel less dramatic effects. The users who don't respond well or strongly would be advised to try a repair cycle of SS-31 first and or combine 5-Amino-1MQ in oral form or subcutaneous injections before writing NAD+ off completely.


That said, it still has its place in optimization protocols for those who do respond well. Reliable performer that continues to deliver for many people, but the injection discomfort is real and something to plan for. Due to pH levels, this one stings going in and then stays stinging for 20 minutes. We’d say it’s similar to a bee sting. The buffered versions do help reduce the sting and also make the peptide last longer. Some would argue the buffered versions feel a bit stronger, but that might vary per vendor. We recently reconstituted a vial using bacteriostatic sodium chloride as a test to help reduce the sting due to it being isotonic, but not enough time has really passed to conclude a full report. We think the sting or injection site reaction has improved, but we’ll comment further on this after we get a larger sample size.
