What is ARA-290?
ARA-290 is a synthetic 11 amino acid peptide derived from the tissue protective domain of erythropoietin (EPO). Its sequence is Pyr-Glu-Glu-Leu-Glu-Arg-Ala-Leu-Asn-Ser-Ser. It is also known as cibinetide and helix B surface peptide (HBSP). Its molecular weight is 1257 Da.
If you have heard of EPO, you know it as the hormone your kidneys produce to stimulate red blood cell production. Athletes have abused it for decades to increase oxygen carrying capacity. But EPO also has powerful tissue protective and anti-inflammatory effects that are completely separate from its blood building activity.
The problem is that when you give someone EPO for tissue protection, you also get all the dangerous blood effects: elevated hematocrit, increased clotting risk, hypertension, and cardiovascular complications.
Researchers identified that EPO's tissue protective effects are mediated by a different receptor than its blood building effects. They mapped the specific region of the EPO molecule responsible for tissue protection, the helix B surface domain, and engineered ARA-290 to mimic just that region.
Why ARA-290 Matters
The result is a peptide that activates tissue repair and calms inflammation without touching red blood cell production, hematocrit, blood viscosity, or blood pressure.
ARA-290 has been studied in multiple Phase 2 clinical trials across sarcoidosis-associated small fiber neuropathy and diabetic neuropathy, giving it a more substantial human evidence base than many experimental repair peptides.

Small Fiber Neuropathy Relief
The strongest clinical evidence comes from neuropathy studies. In sarcoidosis patients, ARA-290 produced a 28% reduction in Small Fiber Neuropathy Screening List scores compared to 9% for placebo.
Nerve Fiber Regeneration
Corneal confocal microscopy confirmed increased nerve fiber area and density after 28 days. GAP-43 positive regenerating nerve fibers increased significantly in the 4mg dose group.
Metabolic Improvement
In type 2 diabetic patients, ARA-290 improved HbA1c by 0.21%, improved cholesterol to HDL ratio, increased HDL, and decreased triglycerides.
Pain & Function
ARA-290 improved pain and physical functioning dimensions of quality of life assessments. The 6 minute walk test improved by 18.7 meters versus a 15.1 meter decline in placebo.

ARA-290 reduces pro-inflammatory cytokines including TNF-alpha, IL-1 beta, and IL-6 through NF-kB inhibition without broad immunosuppression.

A 2021 study showed cibinetide inhibited osteoclastogenesis and increased cortical and trabecular bone mineral density after one month in mice.
