Eloralintide

Loaded question. My short answer is yes, I think Elora could be strong for appetite suppression, but I haven’t seen a head-to-head trial showing it beats Reta. It’s in the amylin lane like Cagri, though they aren’t the same drug. Lilly is studying it alone and with Tirz. Personally, I’d buy Cagri first—I’m not eager to give Lilly more money.

I wouldn’t call Elora and Cagri the same thing. Elora’s weight-loss numbers look strong, and the side effects look encouraging. But those were separate trials, so I don’t think we can say yet that it produces more weight loss or fewer side effects than Cagri.

Same amylin pathway doesn’t mean same drug. Elora is more selective for AMY1, while Cagri has broader receptor activity. Whether that translates into better appetite suppression for people is still an open question.

Fair correction—I meant the same general amylin lane, not identical drugs. My point was about appetite suppression. Reta plus Cagri may hit more pathways on paper, but counting receptors doesn’t prove that stack works better or is easier to tolerate.

On September 30, Lilly reported the Phase 2b in 367 adults with overweight or obesity and type 2 diabetes (NCT06603571). At 48 weeks, efficacy estimand, top dose (eloralintide 9 mg + tirzepatide 15 mg):

Weight: 23.3% (54.1 lb) vs 14.8% on tirzepatide 15 mg alone

A1C: −2.9% vs −2.4%, from a baseline of 8.1%

Eloralintide alone topped out around 12.3%

The catch is dropouts. Discontinuation for adverse events was 10.8–27.0% on the combo, 2.9% on tirzepatide, and 16.7% on placebo, mostly GI during dual escalation. They injected the two drugs separately. Lilly says Phase 3 of a co-formulation with a slower escalation starts in Q4 2026.

I tend to agree. It definitely is not cheap, but once users start doing micro doses we'll see if those sides get diminished, which I suspect they will.
